DANISH trial showed that absolute benefit of ICDs in typical and well-treated HF patients without NIDCM might be small. Younger patients might receive most benefit.

ICDs are not required for every patient with NIDCM. ICD should only be targeted for patients who are at SCA risk despite optimal medical treatment and CRT. CRT-D is useful in patients with ICM with/without VA and in DCM with h/o VA/SCA. CRT-P alone may be adequate in DCM without history of VA and at time of PG replacement in super responders.
